Our Approach

Patient-Centered & Outcome Oriented
At MCNR, the patient is at the center of our care. From admission to discharge, we strive to maximize every patient’s independence, improve their quality of life, and support their recovery goals.

Our highly trained staff provides care that is warm, personal and comprehensive. We take the time to understand our patients’ specific needs and respect their aspirations.

• Seven day a week, 24-hour a day admissions, providing patients with round-the-clock access to comprehensive care.
• Individualized treatment plans that ensure the right balance of nursing care, rehabilitation and other therapies.
• Discharge planning that begins on admission. The nurse and therapist meet with patients and their families to assess the patient’s needs and establish a preliminary plan of care. Early planning assures that treatment and recovery goals guide each patient’s care from start to finish.

Coordinated
The backbone of MCNR’s approach to patient care is our Case Management Team. A nurse case manager and social worker are assigned to each individual, acting as the “quarterback” of the patient’s care team. The role of the Case Management Team is coordination of the day-to-day care plan, ensuring that patients receive the optimal level of service. In addition, they -

• Serve as liaison with other MCNR staff.
• Develop the discharge plan in consultation with the patient, the family and the insurer.
• Coordinate with all outside agencies and community resources

Multi-Disciplinary
MCNR’s dedication to providing the highest quality of care to our patients is reflected in our integrated approach to treatment. Led by the nurse case manager and social worker, the Care Team meets weekly to review the treatment plan and discuss its effectiveness in helping patients reach their goals. The team is made up of all staff involved with the patient’s care including:

• Nursing Services
• Case Management Team
• Social Services
• Physical therapy
• Occupational therapy
• Speech therapy

Committed to Families
Our top priorities during a loved one’s recovery are staying in close touch with families and providing support.

• Patients and families are invited to attend regular or scheduled care planning meetings. The purpose of these sessions is to facilitate a dialogue between family members and our staff.
• The patient’s nurse case manager, social worker and interdisciplinary team review progress towards goals, prepare for the transition home, and are readily available to provide updates and answer questions.
• Caregiver education programs prepare family members to help care for their loved ones at home.
